We will work behind the scenes to find you the right supports to meet your specific goals and needs. We stay in regular contact with you and we work with you to see what is working and what you might want to change.
We support you during the plan review process and if your plan isn’t working or your needs have changed, we also support you through an internal plan review process.
Our role is to support you to achieve your goals and make sure you are getting the most out of your plan.
