Complex support specialists
We are an NDIS registered provider and provide both Support Coordination and Specialist Support Coordination.
We are experts in our chosen field – supporting people through their NDIS journey and complex care needs. We work closely with families, carers and care teams and provide support throughout Queensland, NSW and Victoria.
This includes neurological conditions, brain injury and spinal injury, hospital discharge, housing supports (SIL/SDA), participants in residential aged care (YPIRAC), complex mental health, youth at risk and those in the justice system.
We are here for you on your NDIS journey and we are passionate about making a real difference in the lives of others.

Your dedicated Support Coordinator will provide you with resources to assist you and your family and carers.
We will connect you with the right allied health and care support specialists unique to your condition.
You have complete flexibility, freedom and choice for your care team.
Complex care specialists
We are experts in our chosen field and we are committed to ensuring you and your family are well supported.
We will work with you to manage your providers and work to ensure you have the right aids and equipment available to you.
We will support you throughout your NDIS plan, from implementation to plan review.

We work with you to find creative solutions to meet your needs.
We will ensure you are connected and valued in your community – connecting you with the right service providers and community supports in your local area.
When is it available?
Generally you engage a support coordinator when you have your NDIS plan approved.
You have complete flexibility, freedom and choice over which support coordinator you want to use at any point in your plan.
Is it in my plan?
If you have Support Coordination it will be in your plan under Capacity Building Support.
You can be plan managed and also funded for support coordination – these are two separate services funded by the NDIS.

What does a Support Coordinator do?
Support you to understand, maximise and implement the funded supports in your NDIS plan
Link you to community, mainstream and other government services
Support you to build your ability to exercise choice and control
Connect you with service providers
Make service bookings and help with service agreements
Maintain effective, timely and transparent relationships with all service providers
Communicate with the NDIA and providers with you and on your behalf
Assist you in preparing for your NDIS plan review
Help in times of crisis to resolve points of crisis and develop capacity and resilience
